Case 1:

A 4 year old girl is brought in by dad for her well visit. Her height and weight are both well above 99th%ile, BMI is also above 99th%ile. Dad says child is active, eats a healthy balanced diet. “She’s a kid – she occasionally has chips and cookies, and sometimes we go to McDonald’s. She looks just like all the other kids in her class.”

  • What are you concerned about? 
  • What tests, if any, do you want to order? 
  • What do you advise the family?

You receive the results of the patient’s labwork. Her TC is 185, TGs 150, LDL 130, HDL 30.

  • Discuss the interpretation of these results. What is your plan?

Case 2:

An 18 year old male comes to see you for his annual sports physical. His family history is significant for his father who had a fatal MI at age 42. The patient’s BMI is 22 and he is active, playing on the school soccer and track teams.

  • What tests, if any, do you want to order?

You receive the results of the patient’s labwork. His TC is 220, TGs 80, LDL 180, HDL 38. The other labs you ordered are normal.

  • What is your plan? 
  • How do you counsel this patient? 
  • Can he play sports? 
  • If his labs had been normal, when would you recheck his lipid panel?
